Description
Tadalafil (Megalis) 20 mg tablet is a phosphodiesterase-5 (PDE5) inhibitor that is commonly used for the treatment of erectile problems. This medicine also treats pulmonary hypertension, a type of high blood pressure affecting the arteries in the lungs.
Tadalafil works by directly inhibiting the type 5 (PDE5) phosphodiesterase enzyme, which is responsible for cyclic guanosine monophosphate (cGMP) smooth muscle metabolism. During sexual arousal, the inhibition led to a rise in cGMP levels, causing smooth muscle relaxation and increased blood flow to the corpus cavernosum of the penis, resulting in an erection. In the pulmonary vascular area, it also relaxes the smooth muscle, causing vasodilatation and decreased lung vascular resistance.

How To Take Tadalafil (Megalis) 20 mg Tablet
This medicine may be taken with or without food.
Try to take it at the same time each day. It must be taken regularly for it to be effective.
Take exactly as directed by your doctor or according to the instructions on the label. Do not take more or less than instructed by your doctor. Do not stop taking it unless instructed by the doctor.
Dosage of Tadalafil (Megalis) 20 mg Tablet
Adult: Erectile dysfunction: Initially, 10 mg as needed, at least 30 minutes before sexual intercourse, may be increased to 20 mg depending on response. Alternatively, 5 mg once daily, taken each day simultaneously, may be decreased to 2.5 mg, depending on response. The maximum dosing frequency is once every 24 hours.
Benign prostatic hyperplasia: 5 mg once daily, taken at the same time each day.
Pulmonary arterial hypertension: 40 mg once daily.
Missed Dose of Tadalafil (Megalis) 20 mg Tablet
If you often forget to take your medicine, let your doctor and pharmacist know.
Take the missed dose as soon as you remember. If it is almost time for your next dose, skip the missed dose and return to your usual dosing schedule.
Do not double a dose under any circumstances.
Overdose
If you think you may have used Tadalafil more than the usual dose, please seek medical help immediately.
Contraindications
People with the following medical conditions should not take Tadalafil:
- Hypotension
- Mild to severe heart failure
- Acute recent myocardial infarction
- Other conditions wherein the sexual activity is inadvisable
- Recent stroke
- Uncontrolled arrhythmias and hypertension
- Unstable angina
- Vision loss due to non-arteritic anterior ischaemic optic neuropathy (NAION)
- Severe hepatic impairment (Child-Pugh class C) when used for pulmonary arterial hypertension.

Warnings of Tadalafil (Megalis) 20 mg Tablet
Do not take Tadalafil with the following medicines:
- Alpha 1-adrenergic blockers
- Amlodipine
- Enalapril
- Metoprolol
- Bendrofluazide
- Azole antifungals
- Macrolides
- HIV protease inhibitors
- Cimetidine
- Rifampin
- Carbamazepine
- Phenytoin
- Phenobarbital
Potentially Fatal:
- Organic nitrates
- Riociguat

How Does It Work?
Tadalafil acts by selectively inhibiting the cyclic guanosine monophosphate (cGMP) metabolism in the smooth muscle. The inhibition resulted in a rise in cGMP levels, causing smooth muscle relaxation and increased blood flow during sexual stimulation into the penis, thereby creating an erection. In the pulmonary vascular bed, it also relaxes the smooth muscle, causing vasodilatation and decreased vascular pulmonary resistance.
